211 Counts is a data dashboard to get real-time searchable data in simple-to-use format. Using 211 Counts, you’ll find a snapshot of community-specific needs displayed by ZIP code, region or call center as recently as yesterday, enabling you to easily check trends, make comparisons and share information.
As part of our new 211 Counts partnership, our 211 data can be used to create customized social needs assessments for your community called Calls to Action reports.
These reports identify social determinants of health and social needs trends in your community or catchment area, and recommend evidence-based interventions for the greatest potential impact on health. Calls to Action reports are customizable and tailored to your organization's objectives. The reports can integrate your organization's data alongside 211 data to provide a complete portrait of social determinants of health or social needs in your community.
